Waiver / liability release

I acknowledge that I am the member or the responsible party for the member. By signing this I agree to the following terms of membership:

1. All membership, registration fees and product purchases are non-refundable and non-transferable.

2. Cancellations require a 30-day notice from the date the membership is due.

3. All athletes will adhere by all rules and guidelines set by USA Boxing, BadApple Boxing and it's Coaching staff.

4. Schedule is subject to change at any time.

5. Gym will close for Team BadApple Competitive travel and major holidays.

6. BadApple Boxing reserves the right to refuse service to anyone or cancel a membership at any time without a refund.

Release of liability:

I {name} understand that BadApple Boxing LLC is a physical fitness center and contact sport facility. The use of this facility is at your own risk. I acknowledge that some bodily injury may occur as a result of participating in boxing, mma, wrestling, ju-jitsu classes or while using equipment at BadApple Boxing. I release BadApple LLC, it's coaches and affiliates from all liability.

Automatic Payment Authorization:

I {name} self or legal guardian of athlete, authorize BadApple Boxing LLC to automatically debit the card on file on a monthly basis for gym dues. Payments will automatically debit on the day your membership was established. Example, register of the 12th of the month, means autopay on the 12th of every month.
