BadApple Boxing LLC – Membership Agreement

I acknowledge that I am the member listed below or the responsible legal party for the member. By completing this registration agreement, I confirm that I have read, understand, and agree to the following terms and conditions of membership with BadApple Boxing LLC.

Membership Terms & Policies

1. Non-Refundable & Non-Transferable:
   All membership dues, registration fees, and product purchases are non-refundable and non-transferable under any circumstances.

2. Membership Billing & Late Fees:
   Membership dues are billed on the 1st of each month. Payments not received by the end of the 3rd day of the billing cycle will be subject to a $10 late fee.

3. Automatic Payments & Cancellation Notice:
   Automatic payments will continue to process as scheduled unless a minimum of 30 days’ written notice is provided to cancel membership or stop billing. Failure to provide proper notice may result in additional charges.

4. Rules & Conduct:
   All athletes and members must adhere to the rules, policies, and guidelines established by USA Boxing, BadApple Boxing LLC, and its coaching staff at all times.

5. Schedule Changes:
   Class schedules, hours, and programming are subject to change at any time without prior notice.

6. Closures:
   The gym may close for Team BadApple competitive travel, special events, and major holidays.

7. Right to Refuse Service:
   BadApple Boxing LLC reserves the right to refuse service, suspend participation, or cancel any membership at its sole discretion, without refund.

Release of Liability

I, {Name}, understand that BadApple Boxing LLC is a physical fitness facility and contact sport training environment. I acknowledge that participation in boxing, MMA, wrestling, jiu-jitsu classes, or the use of equipment involves inherent risks and may result in injury.

I voluntarily assume all risks associated with participation and hereby release, waive, and discharge BadApple Boxing LLC, its owners, coaches, staff, and affiliates from any and all liability, claims, demands, or causes of action arising from injury, loss, or damage sustained while participating in activities at the facility.

Automatic Payment Authorization

I, {Name}, as the member or legal guardian of the athlete, authorize BadApple Boxing LLC to automatically debit the card on file for monthly membership dues. Payments will be processed on the 1st of the month, unless a specified date was agreed upon by both parties. 

Acknowledgment

I confirm that I have read, understand, and agree to all terms outlined in this agreement.
